From the moment someone spots it until it is verified that it will not happen again. Each stage notifies its owner, in the app and by email.
The process owner classifies it (type, severity, ISO 9001 clause) and decides who analyzes it and by when.

Notifies the analysis teamCorrective actions to remove the cause, plus improvement actions when needed. Each one is a task with an owner and a due date.
Notifies each ownerOn the planned date it is reviewed with evidence. If it was not effective, it goes back to analysis; if it was, it is closed.

The code is generated automatically, each person signs their part and, once approved, it is distributed to the people who use it. Every read is confirmed, with date and version.
